Health exchange has developed an innovative and unique model for promoting, supporting and maintaining positive behaviour in terms of health and well-being. Our approach is holistic and focuses on creating the context for a dialogue around health and well-being with individuals that values the contribution and knowledge that the individual is able to bring to their health and well-being and offers tools, support, resources and information that enable individual to develop a personal solution to their health and well-being needs. Health exchange is built on the principle that local people are the best advocates for health and well-being within their local community and therefore we operate a policy of open recruitment, development, traineeship and promotion of individuals who are representative of their communities. In order to strengthen the sustainability of our approach Health exchange delivers services with and through community-based organisations who are trusted by the communities they serve. Our approach is based on building clusters or partnerships of community-based organisations working and trading together to deliver services that are both personalised and to a quality standard. We underpin cluster working with targeted social marketing, effective quality systems and professional contract management. Working in partnership with a local social enterprise support organisation we strengthen the capacity of the smaller community based organisations to trade and deliver services of world class quality to meet the needs of those communities and individuals deemed hard to reach.
