Salford Primary Care Together is a Community Interest Company established by Salford GPs to help local people live healthier, happier and fulfilling lives. With a focus on enhancing local health and social care services, the organisation collaborates with all of the city’s GP practices within five designated neighbourhoods: Broughton, Eccles and Irlam, Little Hulton and Walkden, Ordsall and Claremont, and Swinton.
The mission involves facilitating and advancing collaboration among GP practices, encouraging them to consolidate services, staff, and resources. By forging connections between general practice and various professionals in local neighbourhoods—including district nurses, social workers, mental health workers, voluntary organisations, and community groups—the initiative aims to strengthen and enhance all GP practices in the designated neighbourhoods.
The ultimate goal is to provide people with improved access to advice, expertise, and support, fostering independence. Through this initiative, individuals have the opportunity to consult a broader spectrum of health professionals and engage in a diverse range of activities within their local area. The overarching objective is to reduce hospital admissions and visits by ensuring that a comprehensive set of skills and support is readily available within the community where individuals reside.
